today i had a really great day.
after my PBL with my new group, Group I..i went to the hospital with Angel, my tutemate
from India. And then later in the afternoon we
headed to the hospital to have our first abdo exam!!
on a real patient!
it was awesome. i cant really mention the details due
to confidentiality issues but i can tell you that
i had a really interesting day. We meet patient who had ascites (fluid in the abdo)
and also a patient who was jaundiced! he was so yellow he looked like a lemon.wee~
it was great, really, because our previous working problem
was on a patient with jaundice!
I could related everything that was going on.bile duct obstruction.
and i got this sense of enthusiasm because finally i was able to see what i was reading on all this while.
and guess what?
my clinical tutor was so nice, he allowed us to watch a colonoscopy procedure!!
my first one!!
yay~

and then i became the observer for the history taking sessions done by Angel & Mitti.
i became the critic.the mean one. like Simon Cowell. hehehe.
I realised that most of us. th emedical students tend
to 'squeeze' info from the patient regarding their history of presenting illness.
and along that process they forget that patients are humans,
they have feelings. they are not robots. The illness does affect their emotions.so its important that we acknowledge what they are feeling.
Let them talk about their life. about how the illlness is affecting them. talk to them just like how you would want doctors to treat your parents.
thats what i do. i imagine how i would want doctors to talk to umi & walid.
but dont push them so hard. if they dont open up, do it slowly, they eventually will, just like a patient we had just now. He was so solemn.
it was obvious he din wanna talk about his illness. and any further inquiries into the illlness
made him look very restless.so i decided that more personal issues should be discussed, like about his family.Then only he started to smile when we made jokes and all.
then one amazing thing happened, he opened up to us.
he told us that the illness could have been caused because of his wife's death. he said that after that he felt that life was useless & he gave up on life.
what he said then , really opened my eyes. illness or sickness isnt always caused by infection by some deathly virus or anything.sometimes, it can be caused by heart ache. a broken heart.
so as im writing this , a reminder for myself and my fellow med- frens,
look deep into the patient. talk to them. seek the root cause of the disease.
and its never wrong to talk about the matter of the heart.
